Our F.I.L.M. photographers gift family portrait sessions to cancer patients and terminally ill patients across the U.S. and abroad.

To request a session or to nominate someone in your life for a F.I.L.M. Family Session please fill out the request form below. By filling out this information you are agreeing to be the point of contact between the patient, and The F.I.L.M. Project. If you are a family friend, coworker, etc. and are not the person to coordinate the session with us, but would still like to nominate a patient and their family, please first speak to the family and help them through the nomination process.

Once you have filled out this request form, a questionnaire will be immediately emailed to you using the email address you provided (Please verify your email is typed correctly and check your spam or junk folder for this, as sometimes it is misdirected. If, for some reason you do not receive this questionnaire please contact us immediately at sessions@thefilmproject.net ) This questionnaire will help us not only find a photographer in your area, but it will help us to determine the urgency of your request or nomination.

When filling out the information below, be sure to indicate your city and state in the "Event Location" field.  This will allow us to locate a photographer in your area with whom you can schedule your Film Project Portrait Session.  If there is a family reunion, fundraiser, or other family gathering scheduled please provide us with the date in the "event date" field. If there is no "event date" please use today's date.
